Sarvimo Forms: overview
Build surveys, sign-up forms, quizzes and applications, share them with a link, and collect responses.
Sarvimo Forms lets you build a form, share it with a link (or embed it), and watch responses arrive. Beyond the basics it adds things most form tools do not: a time-picker that books a meeting, a PDF copy of every response, drop-off analytics, a one-question-at-a-time layout, and turning a fillable PDF into a form.
Create your first form
- 1Open My forms and choose New form.
- 2Start from scratch, pick a template, let AI generate one from a description (paid plans), or turn a PDF into a form.
- 3Add questions in the Questions tab. Click a question to edit it; use the palette to add more.
- 4Check it in the Preview tab. Nothing is saved from a preview.
- 5Open Settings to choose who can respond, add a thank-you message and more.
- 6Copy the link and share it. Responses appear in the Responses tab.
The four tabs

| Questions | Add, edit, reorder and delete questions. Set validation, options and logic. |
| Preview | Try the form as a respondent, on desktop, tablet or phone width. Nothing is saved. |
| Settings | Access, schedule, layout, thank-you screen, quiz mode, appearance, webhook, Sheets and connectors. |
| Responses | Stats, drop-off funnel, a searchable table, a detail drawer and exports. |

There is no monthly cap on responses. If you downgrade, your forms stay viewable and exportable for a 30-day grace period before locked forms are removed.
