Turn a PDF into a form
Upload a fillable PDF, get an online form, and download each response as the completed PDF.
Have a fillable PDF such as an application, consent or registration form? Upload it and Sarvimo turns each field into a question. When someone responds, you (and they) can download the original PDF with their answers filled in.
How to do it
- 1Open New form and choose Turn a PDF into a form.
- 2Pick your PDF (up to 5 MB).
- 3Review the questions created for you and adjust them in the builder.
- 4Share the form. Each response can be downloaded as the completed PDF from the response drawer.
Field mapping

Heads up:This works with fillable PDFs, ones you can type into in a PDF viewer. A flat scan has no fields to import. Text in the filled PDF supports Latin characters only for now. Password-protected PDFs are not supported.
